Tribal Assistance Programs
The following is a list of Tribal Assistance Programs for enrolled members of the Iowa Tribe of Oklahoma. The application forms are now online for you to download, printout, fill-out, and send to the Tribal Assistance Department.

Tribal Assistance Programs

Must be an enrolled member of The Iowa Tribe Of Oklahoma.

All requests for assistance must be submitted on an official tribal assistance form. NO Faxes allowed, must be mailed or hand delivered.

Forms must be complete, legible, and include all required documents.

All request for assistance shall be approved by the Tribal Administrator.

All receipts must be from January 1st through December 31st, with Date and Name of Vendor.

Any fraudulent activity will be subject to prosecution.
-
Pledges for household assistance are allowed twice a year per household.

Incomplete applications will not be processed.
-
Please allow 10 business working days for processing.
Burial Assistance
Burial assistance is provided to assist in the cost of funeral and traditional ceremonial expenses at the time of death of a Tribal Member.

The decedent must have been an enrolled member. The person completing the application on behalf of the decedent family must be the next-of-kin. Cases of dispute will be determined by action of the Business Committee.

The application must be supported by presentation of the death certificate or other official documentation of death provided by a funeral home. If the funeral home is unable to provide documentation in a timely manner, program staff may verify the information via telephone so that the family's portion of the assistance may be paid. Written documentation must be received before release of the check to the funeral home.

The Tribe will pay $5,000 directly to the funeral home and up to $1,250 to the family to provide for additional funeral expenses, or the family may request that all funds be paid to the funeral home.

Burial Assistance – Infants
Assistance is provided to assist in the cost of funeral expenses at the time of death of an infant of an enrolled Tribal Member

The infant decedent must have been the biological child of an enrolled tribal member, who, if they had survived, would have been eligible for membership with the Tribe.

Dental/Eyecare Assistance
Assistance to supplement cost for dental and optical wellness.
This assistance includes: Dental exams and treatment, orthodontic treatment, partials and dentures.
Eye exams and treatment, eye glasses and contact lenses
Eligibility Time Period
Yearly / January 1-December 31
Amount of Assistance
$2,000

May be used for services of a dental or optical provider.

Applicants may elect to have funds paid directly to the dental or optical provider or may be reimbursed for expenses.

For provider payments, applicants must be pre-approved before the medical exam. Following approval, a letter of authorization will be issued to applicant to schedule an appointment. The letter must be used within 60 days or the applicant must re-apply. (Deadline Oct. 31st for Letter)

For reimbursements, applications must be supported by an original receipt before funds are released to the applicant.

Education Incentive – Higher Education
Assistance is provided to promote higher education achievement.
Eligibility Requirements: Enrolled in an accredited university or college courses leading to a degree program.
Eligibility Time Period
Yearly / January 1-December 31
Amount of Assistance

Applications must be supported by an original grade report, within 120 days after the close of the semester and /or the receipt of grades.

Grades must be documented by a certified statement on a university letterhead or university generated grade report, and an official transcript.

The Iowa Tribe reserves the right to independently verify the submitted grades.

incentive is calculated by multiplying the dollar amount, which is based on current grade point average(GPA), times the credit hours passed per semester.

High School Senior Assistance
Assistance is provided to offset the added expenses for graduating seniors.
Eligibility Requirements: High School Student anticipating 12th Grade Graduation.
Eligibility Time Period
One Academic Year / August - June
Amount of Assistance
$2,000

Allotted $1,000 (per semester).

This includes: Yearbooks, senior; cap and gown, announcements, pictures, class ring, computer, senior trip and college testing or other related graduating expense.

Application must be supported by a statement from the high school on an official school letterhead stating the date of planned graduation and signed by a school official.

The Iowa Tribe reserves the right to independently verify the submitted statement.

Applicants may elect to have funds paid directly to the vendor or may be reimbursed for expenses.

For reimbursements, applications must be supported by an original receipt before funds are released to the applicant. Receipts from the academic school year are accepted.

Education Incentive
Assistance to promote educational achievement for completion of the 8th grade, 12th grade and GED or Vocational Training.
Eligibility Requirements: Enrolled in a accredited public or private school, GED, or Vocational education program.
Eligibility Time Period

School Expense Assistance
Assistance to encourage and promote education for youth.
Eligibility requirements: Enrolled in a accredited public, private school or licensed child care facility.
Preschool through 12th Grade High School Senior.
Eligibility Time Period
Yearly / January 1-December 31
Amount of Assistance
$1,200

Allotted $600 (Jan. - June) and $600 (July - Dec)

This includes: Year books, academics, extra-curricular activity participation and supplies, clothing, pictures, class rings, lunches, supplies, lab fees, testing fees, graduation expenses, sports activities, summer academic fees, science projects, computers. (ONLY - school age children)

Gift cards may be requested, but receipts must be returned to ITO in two(2) weeks from the date the card was received. NO EXCEPTIONS; If receipts are not returned to the Iowa Tribe in a timely manner, the applicant will no longer be eligible for gift cards.

Gift cards must be solely used for the purpose of the application.

Child care for ages 0 through 13 years of age. Must be a licensed child care facility.

Applicants may elect to have funds paid directly to the vendor or may be reimburse for expenses.

Gift card Application
Elder´s Assistance
Assistance to promote the quality of life for Tribal Senior Citizens.
Eligibility Requirements: 55 yrs. or older
This includes ONLY: Household expenses & repairs, personal cost, appliances, furniture, medical expenses, loan payments, car payments, cable and telephone.
Eligibility Time Period
Yearly / January 1-December 31
Amount of Assistance
$2,000

Allotted $1,000 (Jan. -June) and $1,000 (July - Dec.)

Application must be supported by estimated bids, invoices, and original receipts.

Gift cards from Wal-Mart may be requested with an itemized list of proposed expense.

Applicants may elect to have funds paid directly to the vendor or may be reimbursed for expenses.

For reimbursements, applications must be supported by an original receipt before funds are released to the applicant.

Hearing Aid Assistance
Assistance to supplement cost for hearing wellness.
This assistance includes: Hearing exams and purchasing a hearing aid.
Eligibility Time Period

$2,500 for the initial setup and $1,000 every year for follow up maintenance.

Household Assistance
Assistance to supplement the cost of household expenses.
Eligibility Requirements: 18yrs. of age or older.
This includes ONLY: Personal/property taxes, property/rental insurance, utilities, rent and mortgage payments.
" While your application is in process, please continue to make payments on your utilities/rent. The Iowa Tribe Of Oklahoma is not responsible for late charges that may incur or cut-off notices."

All utility payments are paid directly to the vendor

Rent, mortgage, property taxes and property/rental insurances payments may be reimbursed with current receipts or paid directly to the vendor.

Applications must be supported with an invoice, a current utility bill, current lease agreement, or receipt of payment.

Pledges to vendors may only be made twice a year per household and cannot be canceled.
-
For all new vendors, a W-9 form must be submitted in order to process payment.

Tribal members who are incarcerated may apply and have assistance placed on commissary account. Application must include their identification number.

Prescription Assistance
Assistance is supplement cost of prescription drugs to promote health and wellness. medications must be prescribed by a medical doctor and not covered by any insurance or Indian Health Service.
