Community Health Representative
The CHR program is responsible for developing and coordinating all services, activities and referrals for eligible Native American clients residing within the service area of Payne, Logan, Oklahoma, and Lincoln counties enabling individuals to gain access to comprehensive health care.
The CHR program is responsible for the following services:
- Provide community health education (CPR/First Aid/AED classes and Blood Born Pathogen training)
- Conduct home visitation with eligible clients as needed
- Provide case management on all clients
- Coordinate with clinic staff for outpatient services
- Offer blood pressure and blood sugar screening to all community members on a regularly scheduled basis
- Provide basic health care and monitor health status for eligible clients
- Lunch and Learns at the WhiteCloud Building throughout the year on various Health Topics
- Yearly Health Fair
- Drive-Thru Flu Clinic
- Disbursement and monitoring of medical supplies and pharmaceuticals to eligible clients
