The CHR program is responsible for developing and coordinating all services, activities and referrals for eligible Native American clients residing within the service area of Payne, Logan, Oklahoma, and Lincoln counties enabling individuals to gain access to comprehensive health care.

The CHR program is responsible for the following services:

• Provide community health education
• Conduct home visitation with each eligible client a minimum of once per month
• Provide case management on all clients
• Coordinate with clinic staff for outpatient services
• Offer blood pressure and blood sugar screening to all community members on a regularly scheduled basis
• Provide basic health care and monitor health status for eligible clients
• Disbursement and monitoring of medical supplies and pharmaceuticals to eligible clients
