Health Programs
Diabetes Program: This program's primary objective is to reach family members
of diagnosed diabetics to provide support, information, and educational programs. For
example: diabetic support group, quarterly nutrition education, special luncheons held
quarterly, a quarterly newsletter, Heartland Share Promotion and distribution, and an
annual diabetic camp.
This program's secondary objective includes preventing the complication of diabetes,
early diagnosis of people with diabetes, and addressing emotional aspects. For example:
Regular individualized meetings with a registered dietician, Foot risk assessment by CHR,
Pawnee Health Center, and Diabetic Coordinator, Free glucometer and glucose strips, and
Quarterly Case Management to be completed through home assessments and office visits.
The programs tertiary prevention is for diabetics who are considered high risk in feet and
in blindness. Services include Quarterly referrals to appropriate medical personnel,
coordination of transit, and to provide referrals for foot and eye glasses as necessary
utilizing all available funds to support these services.
Title VI Nutrition Program: This program provides noon meals Monday through
Friday 11:30 am - 12:30 pm for Native American seniors and disabled family members.
Eligible individuals need to live in portions of Payne, Lincoln, and Logan counties.
The noon meal can be enjoyed at the Iowa Tribal grounds in the White Cloud Building
or it can be delivered to a homeboud person's home. The Title VI services also
include transportation to medical and personal business appointments, scheduled
shopping trips and seasonal activities.
A full time cook is on staff to prepare nutritionally balanced meals meeting 1/3 RDA
requirements. The meals are nutritionally balanced and served with table cloths,
silverware, and china. The meals delivered are in insulated containers to maitain
warmth for home delivery. A full time outreach worker is on staff to assist with
finding aging resources available: income assistance, aging support services, and
general services to assist elders in maintaining independence within their home.
In addition, a full time Title VI Coordinator is on staff to manage and oversee
senior nutrition and out reach services.
Community Health Representative (CHR): The CHR program supplies
direct patient care, screening and facilitation of other services and referrals
to other Para professional health care providers.
Substance Abuse Program: offers assistance to families and
individuals who are dealing with living problems associated with drug abuse. The
program offers education and counseling services. The program goal is to assist
the individual in adopting and maintaining a chemical free lifestyle. This
program is open to the community at large on a fee-for-service basis. The focus
is on DUI, assessment and and psychological counseling.
Tribal Youth Program: focuses on preventing substance abuse and
gang involvement. This is an activity-based program involving various events such
as foot races, basketball tournaments, etc.
The Fitness Center provides a physical training facility that
consists of free weights, aerobic equipment and a dry sauna. The focus is on
prevention. There is a certified fitness instructor on staff. A nominal fee is
collected for non-Indian use.
Massage Therapy is open to everyone and is available one day a
week from Noon till 5 PM. Meditation sessions are also available. Management would
like to expand both of these services and market to the general public.
