Diabetes Program
GOAL 1: to prevent or delay the onset of diabetes by raising awareness and providing education to prevent type 2 Diabetes
GOAL 2: to prevent or delay the of onset complications of diabetes by raising awareness and providing education to prevent amputations, blindness, and kidney disease
GOAL 3: to promote healthy lifestyle and diabetes self-management
Program Participants need to complete a needs assessment at Perkins Family Clinic. Call and make an appointment for Diabetes clinic or with the Diabetes Coordinator (547-2473).
Program Participants are encouraged to see their physician every 3 months for diabetes check-ups. Your physician may schedule you more frequently depending on your individual needs.
Program participants are eligible for diabetes education, nutrition education, physical activity education, blood sugar meters (glucometers) & supplies. Patients that are active in the program and reside in our service area are eligible for stipends toward shoes, dentures and eyeglasses. Active patients need to have attended diabetes clinic and had appropriate lab work during the past six months and have two downloads from their glucometers.
For patients that have high risk feet, the diabetes program can pay a stipend up to $200.00 for therapeutic shoes/inserts per grant year. For patients requiring eyeglasses, the diabetes program can pay a stipend up to $200.00 for eyeglasses. For patients requiring dentures, the diabetes program can pay up to $250.00 (This is assuming all criteria are met and that there are monies available.)
Our staff includes a Diabetes Coordinator/Nurse and a Dietitian.
We work in a team effort with Perkins Family Clinic and the Tribal Wellness Center.
Diabetes Clinic appointments are scheduled on the first and third Wednesday of each month.
